The Application Process
5 Simple Steps to Get Started
Submit Your Application
Complete and submit the application form below or contact us directly. Provide basic information about yourself and your current housing situation. All information is kept strictly confidential.
Initial Review
Our team will review your application within 2–3 business days. We will assess your eligibility based on our program criteria and current availability.
Intake Interview
If your application moves forward, we will schedule a confidential intake interview. This is a conversation — not an interrogation. We want to understand your needs, goals, and how we can best support you.
Program Orientation
Accepted residents will attend a program orientation where you will learn about our services, expectations, resident handbook, and what to expect during your time with us.
Move In and Begin
Once orientation is complete and all paperwork is finalized, you will move into your new home and begin working with your dedicated case manager to set your personal goals.
Referring Someone
Know Someone Who Needs Help?
Social workers, healthcare providers, community organizations, and concerned individuals can refer someone to our program. Simply complete the application form on their behalf or contact us directly to discuss the referral process. We work closely with referring agencies to ensure a smooth and supportive transition.
